• Professional Current License: RN/LPN/CNA

• HHA: Certificate, Curriculum, PB ID Badge

• Physical Health Statement (“Free of Communicable Disease”) (within 6 months)

• CPR (Approved by AHA or ARC)

• CEU’s:

• HHA/CNA: HIV/AIDS/OSHA, Domestic Violence, Alzheimer’s/Dementia, Assistance with Self-Administration of Medications (at least 2 hrs.), Preventing Medical Error

• LPN/RN: HIV/AIDS/OSHA, Domestic Violence, Alzheimer’s/Dementia, Preventing Medical Error

• Current Driver’s License

• Proof of Automobile Insurance

• Social Security Card

• Permanent Resident Card/Work Permit Card/Proof of Citizenship

• Proof of Professional Liability Insurance

Notification: Senate Bill 252 Emergency Rule Mandates and Treatment Options

On May 11, 2023, Governor DeSantis signed Senate Bill 252 which provides protections from discrimination based on health care choices. Specifically, section 5 of Chapter 2023-43, Laws of Florida, requires the Agency for Health Care Administration (Agency) and the Department of Health to jointly develop standards for the appropriate use of facial coverings in health care settings.
